Just as Vridian and his crew were going somewhere relaxing, in hopes that they won't crash the ship, Viridan somehow managed to do so, again, and into yet another dimension filled with spikes, trinkets, and terminals.
Well, you better go rescue your crew members. Again. 
Why, you might ask? Well, Viridian's done this about a thousand times, and since he has nothing better to do...
(Oh, and in case I didn't say this, this level's a work in progress.)

...Or at least a level that had some time in the making that happened to be 4 years after '5 Stages Of Pain'.
